Survey Configuration
Custom survey sent to requesters upon WO completion.
To create a Survey, click the icon.
All fields outlined in red are mandatory fields to be completed to create a Survey.
- Description: Title of Survey
- Survey Header: This title/header will be at the top of the new survey.
- Active: This makes the survey active for surveyors to complete.
- Frequency Limit (per recipient):
- Category: This is the type of Work order(s) needed for the surveys to be sent.
- Status: This is the status of Work Order(s) needed for the surveys to be sent.
- Email from Address: email address shown in the email from area.
- Email Subject: This is the subject line of the email when the survey is sent.
- Email Body: This is the content of the email for the survey.
- Work Order Fields: This drop down will place the information from the work order which the survey is being sent.
- Once all fields have been completed, click Save.
A progress indicator appears, and a note will display that the survey has been successfully saved. The survey Questions portal appears on the right.
To add question, click the icon.
Type the question and then select Type (Text, Radio Buttons, & Slider).
- Text Box: Allows requester to free type their answer into text box
- Radio Buttons (select one): Allows the requester to choose one answer; type the answer choices one per line as seen below.
- Check Boxes (all that apply): Allows the requester to choose multiple answers; type the answer choices one per line (same as Radio buttons).
- Slider (scale): Allows requester to select a number value; enter number values for the fields:
- Slider Min. Value is the 1st number on the scale.
- Slider Max. Value is the last number on the scale.
- Slider Increment Value is the ratio of numbers. For example if 2 is entered on that line, then the slider will show values of 2, 4, 6, etc.
- Slider Default Value is the number where the slide will be set on the scale.
- Required: Indicates if it is a required question to be answered.
- Active: Indicates if the question is enabled.
Once a WO has been completed and Status, Category, Trade and Priority match, fitting the Frequency Limit, an email will be sent to the requester with a link to the survey to be completed.
Survey Results are stored within CMS and can be found by running the following reports in the Reports module.
